How can I get into a top uni if I would like to change my path?

I'm 21 and recently graduated from a diploma I didn't do well in. Being young I had chosen a course I didn't like out of fear. It was design. Is it even possible to restart get into a top university in US/UK, if I were to try SATs or exams now? I know I can study hard and am willing to do anything.
