Is there an app that allows for questions during a field trip on student phones?

So I'm a student geography teacher and have to prepare a field trip for my fellow students, which means I can't make use of a school printer for a traditional worksheet with questions per location.

So is there an app that allows for both planning a route with stops, as well as ask questions per stop? Sort of like google earth and google forms/lessonup combined? This would be great.
