Diagnostic Reading Assessment questions

Hi, I hope I've posted this in the right forum. If I haven't, please let me know where I should!

My questions involves the Diagnostic Reading Assessment test in public elementary schools. I have tried finding info online about it, but haven't been successful.

Here are my questions: 1) what states is this test required in? Or does it go by county? Are there any states where it is not required? 2) is it required to graduate to the next grade? 3) which grades is it given in? 4) is it required in charter and private schools?
