What are the main reasons/what would need to be done to lessen/close the educational (k-12 and college, etc.) achievement gap between all ethnic groups in the U.S. (and in whatever other countries this is applicable)?

I skimmed the Wikipedia article about educational achievement differences among different ethnic groups in the U.S. and it seems like there are tons of variables and historical reasons for them, but are there any specific causes and needed solutions/policy changes that cause the majority of the gaps and that would, with a probability of success, possibly based on other developed countries' models (like Finland in the PISA exams), close the gaps and thus reduce the amount of bad things that happen daily?
