A little help

Hello Teachers,

I'm a teacher and Ed Tech administrator at my school trying to conduct a school-wide teacher survey of how teachers feel technology is used (or not) and how they feel supported (or not) in the classroom regarding Ed Tech tools. I already have several drafts of surveys prepared but I wanted to see if anyone had a google template, a bank of questions, or a specific question they thought would be good to ask. Maybe even a question that they'd like to be asked, perhaps a question you'd like to ask your ed-tech admin (other than why the printer doesn't work or why the wifi is down... I can't help I'm sorry)

I've spent 10+ years in the classroom and I know what it's like to not have your voice heard and for decisions to be made without input so I'm trying to avoid that with my teachers. Any and all help is muchly appreciated.
