Recently I took the STAR reading and math test, my reading and math tests have both always been extremely high for my age. Hence, I am in the grade above for math since my school does not offer and honors program for math or english. I got the results back for my math assessment and it is quite awful. Almost every single part of it i scored awfully on. It says I am still above 79% of the people in my grade, but I have an extremely awful score. I have a couple of questions and some concerns.

Will this get me kicked out of the advanced class?

Will this impact my ability to take AP classes in the future?
My concerns are that I played contact sports for many years and have suffered multiple concussions from that and not from that. I can very easily get a concussion now since I have had multiple. My most recent concussion was last year. Before then my STAR tests were much above my grade level in math and reading. Now, my math assessment is extremely awful. I have been having issues recalling subjects I had learned and felt like I didn’t know anything on the assessment.
The concussion in question was from when I had taken a pretty serious fall and banged the back of my head on concrete. I don’t remember exactly what I felt like after but I remember everything for the next few weeks feeling very foggy. My academic performance definitely decreased and since then I have had issues in math.
My teacher has also played contact sports and I feel like he would understand this. I have talked to my parents about this but they haven’t seen my score yet. Should I talk to my doctor/teacher/parents? Or should I wait until either my teacher or parents say something.

score of 1137 on the Star Math test is based on the difficulty of the questions and the number of correct responses. The scale has a range of 600 - 1400. This score results in a placement in Level 2, below the target of Level 3. It is higher than 79% of students in 9th grade. All of the stuff I saw on the assessment I do partially remember from last year but I couldn’t figure out how to solve it.

Edit: Does this also make me technically “disabled” and will i be forced to enter special education or a 504 plan? (If the concussion caused serious injury to my brain
