Submitted a grade change form I thought was acceptable, but now I’m not sure…

I was informed by one of my students recently of a grade discrepancy between their HS and college transcript (dual-credit class), where their grade in one of the transcripts was half a letter grade higher than the other. Assuming that it was a clerical error on my part, I kept the higher half letter grade and submitted a grade change form for the other transcript.

Well, here’s the awkward part…I discovered later that apparently the grade that was half a letter grade lower was the original grade and the higher grade was a manual change in the original grade (as for the reason, I’m still trying to figure out).

I’m concerned and a bit frightened as to what to do because I just submitted the grade change form (this is in paper btw) and am not sure if I’d be in deep trouble if I were to let this go (this is an B- to an B grade change). In addition, this student was a hard working individual in my class, though even then, the B obv wasn’t the original grade.

To clarify stuff, B- and B provides different GPA points, so they’re not technically the same grade in the grading system for both HS and college.

I know I probably fucked up and should have asked the student what the original score was instead of assuming that it was my clerical error. In addition, this is the only change I’ve seen, nothing else. Fellow teachers, what do I do…

EDIT: adding in more info, I am in a union (assuming that I might need to contact them to escalate this if needed)
