healthy lifestyles questionnaire

hey y’all!

i’m in a clinical doctorate psychology program and for our psychometrics project we came up with a survey assessing healthy lifestyle choices. in particular we’re aiming to get a better understanding of a specific topic that has very little research done thus far.

i’d so deeply appreciate it if you could help a gal out during her first year of doc program!! it should only take a couple minutes.
