Survey of parents and teachers of 3rd to 5th grade students with ADHD/ADD

I am participating in a research project this summer. If anyone who is a parent of children with ADHD in 3rd/4th/5th grade and/or a 3rd, 4th, or 5th-grade teacher, I would appreciate it if you could complete the survey. There are separate links below for parents and teachers.  

You are invited to participate in a research study conducted by education students from University of Wisconsin – Superior. Your participation is voluntary. 

The purpose of the study is to investigate the perceptions and beliefs of both teachers and parents in how students with ADD/ADHD are supported in the elementary classroom; and analyze the differences between the two perspectives. This project seeks to identify and understand the differences in knowledge of ADHD/ADD in young children and the best practices in supporting their learning, so that students can reach their optimum potential.
