Can someone please show me or tell me what a high school transcript looks like?

I wanna study abroad.

When I apply to a university, it asks me to submit a high school transcript.

So, I went to the high school I attended, and asked to give me a transcript.

(In our country, schools usually don’t give a transcript. To go to university, we just need to submit our matriculation exam marks.)

When I went there and asked for a transcript, they said they don’t know what to do as they don’t know the format. All they can do is searching for my grades and marks that I got at the tests and exams.

They told me to make a form by myself and print it out. Then, they said they will gonna fill the form.

But the problem is that I’ve never seen a high school transcript before. And I don’t know what should include in a transcript.

Is it okay if I just put school grades and the subjects I learnt. What do I need to add more? Help!

Btw when I googled it, some people also said that a high school transcript should be sealed in an envelope. Do I need to do that?
