Practice makes perfect

Ok, big question here for teachers. I think it could have both abstract/theoretical and practical answers.

So, I'm learning HOW MUCH I need to drill things to get students to improve at them. I need to have kids practice everything. I can't expect to do a thing once and have it change. Obvious to most, maybe, but I'm still new.

Ok, so let's imagine- a teacher is excellent, thorough and thoughtful about how they practice and repeat drills for various things. The kids still don't get better. To be clear, this has not happened. I'm just doing a kind of thought experiment.

So if the practice is there, and things don't improve, what is going on?

From a pragmatic standpoint, what should a teacher do?

From a theoretical standpoint, what's going on?

Combining both somehow?
